Abstract


Drug-induced gingival enlargement is an abnormal growth of the gingiva due to adverse drug reaction in patients treated with immunosuppressants, calcium channel blockers, and anticonvulsants. Especially a correlation between phenytoin, nifedipine, cyclosporine and gingival enlargement are found to be evident. A 7-year-old boy came to the Department of Pediatric Dentistry, Yonsei University Dental hospital for gingival overgrowth. He has been taking tacrolimus and amlodipine due to a history of liver transplant and hypertension. Clinical examination revealed generalized severe gingival enlargement in the region of maxillary and mandibular teeth. Panoramic view showed increased thickness of gingiva up to incisal 1/3 portion of mandibular anterior teeth. As there were risks in invasive procedures due to immunosuppressive drugs, regular dental check-up with scaling had been done for a year. At the age of eight, scalpel gingivectomy was performed in both maxilla and mandible under general anesthesia. Gingival enlargement induced by tacrolimus alone is rare, but when taken with calcium channel blocker it might have synergistic effect. The correlation between patient¡¯s periodontal health status with gingival enlargement as well as systemic effect has also been proved, therefore maintaining good oral hygiene would be an important factor to reduce the risk for recurrence.
